Join a leading home healthcare team as a Licensed Practical/Vocational Nurse (LPN/LVN) Responsibilities Include Providing and documenting skilled nursing care under RN supervision in accordance with care plans and physician orders.Assessing patient conditions, reporting changes, and implementing appropriate nursing interventions.Maintaining accurate documentation of activities, assessments, and responses.Educating patients and families on disease processes, self-care, and prevention strategies.Ensuring compliance with safety, infection control, and regulatory standards.Participating in ongoing education and skill development. Qualifications And Skills Current, unrestricted LPN/LVN license in the practice state (compact licenses accepted if transferred within 90 days).Current CPR certification (training available post-hire if needed).TB test within the past 12 months.Reliable transportation and ability to travel to patient locations.Preferred: Six months of recent clinical experience, home health experience, and proficiency in assessments and documentation.Effective communication, attention to detail, time management, and problem-solving skills. Physical And Environment Requirements Ability to lift up to 50 pounds and reposition patients.Prolonged walking, standing, bending, and climbing stairs.Ability to function in diverse environments, including potential exposure to blood and infectious diseases.Strong visual and auditory acuity, and physical responsiveness to emergencies.
