In Home Healthcare LVN - Weekend Shifts

About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting.

Responsibilities

• Delivering and documenting patient care according to care plans and physician orders
• Assessing patient conditions and reporting changes to supervisors or physicians
• Updating and implementing nursing care plans
• Providing patient and family education on disease management and self-care
• Ensuring infection control and maintaining confidentiality
• Participating in ongoing education and compliance with regulations

Qualifications

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification (training available if needed)
• TB test within the last 12 months
• Reliable transportation and ability to lift up to 50 pounds
• Six months of clinical experience preferred; home health experience a plus

• Ability to problem-solve and respond effectively to emergencies
• Proficiency in assessments, documentation, and compliance
• Ability to operate in diverse environments with exposure to biological hazards
• Physical ability to walk, stand, bend, and reposition patients as needed
