About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting, working under the supervision of a Registered Nurse.
Responsibilities
- Delivering and documenting patient care
- Assessing patient conditions
- Implementing and updating care plans
- Recognizing and responding to changes
- Educating patients and families
- Maintaining infection control standards
- Adhering to policies and regulations
- Documenting all care activities accurately
- Providing compassionate patient care
Qualifications
- Active, unencumbered LPN/LVN license
- Current CPR certification
- TB test within 12 months
- Reliable transportation
- Preferred: six months of clinical experience
- Knowledge of documentation, compliance, and safety protocols is essential
