In Home Healthcare LVN - Weekend Shifts

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings for adult patients.

Responsibilities

• Deliver and document patient care according to physician orders and care plans.
• Monitor and assess patient conditions, reporting changes promptly.
• Maintain accurate documentation of assessments, actions, and responses.
• Implement nursing interventions to achieve desired outcomes and prevent risks.
• Educate patients and families on disease management, self-care, and prevention.
• Adhere to infection control standards and professional guidelines.
• Participate in ongoing education and skill development.

Requirements

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification.
• TB test within the last 12 months.
• Reliable transportation and ability to travel.

Skills

• Proficiency in clinical assessments, documentation, and compliance with policies.
• Attention to detail and strong organizational skills.
• Problem-solving and communication skills.

Physical requirements

• Ability to lift up to 50 pounds.
• Prolonged standing, walking, bending, and repositioning patients.
• Visual and auditory acuity, and ability to respond effectively to emergencies.
