In-Home Healthcare LVN - Weekend Shifts

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse. This role involves providing compassionate, high-quality care to adult patients, with flexible scheduling options and opportunities for professional growth.

Responsibilities

• Delivering and documenting patient care according to care plans and physician orders
• Assessing patient condition, monitoring changes, and reporting to supervisors or physicians
• Implementing nursing actions to achieve expected outcomes and prevent risks
• Educating patients and families on disease management, self-care, and prevention
• Maintaining compliance with policies, regulations, and infection control standards
• Participating in ongoing education and skill development

Qualifications

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification and TB test within the last 12 months
• Reliable transportation and the ability to travel
• Attention to detail, strong communication, organization, and problem-solving skills
• Physical ability to lift up to 50 pounds, reposition patients, and perform home care tasks
• Preferably 6 months of recent clinical or home health experience
• Ability to work in diverse environments and respond to emergencies
• Compliance with health-related requirements, including vaccinations and testing as per jurisdictional mandates
