About the role
We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings for adult patients.
Responsibilities
- Delivering and documenting patient care according to physician orders and care plans
- Monitoring and assessing patient conditions, reporting changes promptly
- Administering medications and treatments, maintaining infection control standards
- Educating patients and families on disease management and self-care
- Ensuring compliance with all regulatory and safety protocols
- Participating in ongoing education and skill development
- Providing compassionate, quality care in diverse home environments
- Maintaining accurate documentation and clinical assessments
- Recognizing patient needs and responding appropriately to prevent risks
- Adhering to infection control and safety standards
Requirements
- Active, unrestricted LPN/LVN license in the practice state
- Current CPR certification
- TB test within last 12 months
- Preferred: six months of clinical nursing experience
- Reliable transportation
- Strong attention to detail, organization, communication, and problem-solving skills
- Physical ability to lift 50 pounds, reposition patients, and perform necessary movements
