About the role
We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a homecare setting.
Responsibilities
- Delivering and documenting patient care according to physician orders and care plans.
- Assessing patient conditions, monitoring changes, and reporting to supervisors or physicians.
- Implementing nursing actions to achieve desired outcomes and prevent risks.
- Educating patients and families on disease management, self-care, and prevention.
- Maintaining compliance with policies, regulations, and infection control standards.
- Participating in ongoing education and skill development.
Qualifications
- Active, unrestricted LPN/LVN license in the practice state, with transfer of compact licenses if applicable.
- Current CPR certification and recent TB test.
- Six months of hands-on nursing experience preferred but not required.
- Reliable transportation and ability to travel.
Required Skills And Physical Abilities
- Attention to detail, effective communication, and time management.
- Ability to lift up to 50 pounds, reposition patients, and respond effectively to emergency situations.
- Proficiency in English and capability to function in diverse environments, including exposure to blood and infectious materials.
