In-Home Healthcare LVN - Weekend Night Shifts

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting, under the supervision of a Registered Nurse.

Responsibilities

• Delivering and documenting patient care according to care plans and physician orders
• Assessing patient conditions, recognizing changes, and reporting to supervisors or physicians
• Implementing nursing actions to achieve clinical outcomes and prevent risks
• Educating patients and families on disease processes, self-care, and prevention
• Maintaining compliance with policies, regulations, and safety standards
• Participating in ongoing education and skill development

Qualifications and Skills

• Active, unrestricted LPN/LVN license in the practice state
• Current CPR certification
• Reliable transportation and ability to travel
• Proficiency in assessments, documentation, and compliance
• Attention to detail, strong communication, problem-solving, and organizational skills

Requirements

• Ability to lift up to 50 pounds and reposition patients without assistance
• Prolonged standing, walking, bending, and reaching
• Ability to respond to emergency situations and work in diverse environments, including exposure to infectious materials
