About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting. This role involves providing compassionate care to adult patients, utilizing electronic documentation, and working collaboratively with a multidisciplinary team.

Responsibilities

• Deliver and document patient care according to physician orders and care plans.
• Monitor patient conditions, assess needs, and report changes.
• Implement nursing actions to achieve clinical outcomes and prevent risks.
• Educate patients and families on disease management, self-care, and prevention.
• Maintain compliance with policies, regulations, and safety standards.
• Participate in ongoing education and skill development.

Qualifications and Skills

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification and TB test within the last 12 months.
• Reliable transportation and ability to handle physical demands such as lifting and repositioning patients.
• Effective communication, attention to detail, time management, and problem-solving skills.
• Preferred experience in clinical assessments, documentation, and home health care.
