About the role

Join a healthcare team dedicated to compassionate patient care as a Licensed Practical / Vocational Nurse (LPN/LVN).

Responsibilities

• Provide and document skilled nursing care in accordance with care plans and physician orders.
• Assess patient conditions, monitor changes, and report to supervisors or physicians.
• Participate in updating and implementing nursing care plans.
• Educate patients and families on disease management, self-care, and prevention.
• Ensure infection control and adhere to confidentiality and professional standards.
• Maintain accurate documentation of all nursing activities.
• Deliver quality patient care, observe and evaluate patient status, document activities, and collaborate with healthcare team to achieve optimal outcomes.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification and TB test within the past 12 months.
• Preferred: Six months of clinical nursing experience; home health experience is a plus.
• Strong attention to detail, organization, time management, and effective communication skills.
• Physical ability to lift up to 50 pounds, reposition patients, and respond to emergencies.

Additional requirements

• Reliable transportation and ability to travel as needed.
• Ability to operate in diverse environments, including exposure to bodily fluids and infectious materials.
• Continued education as required by state regulations.
