In-Home Healthcare LVN - Adult Patients (Afternoon/Evening Shifts)

About the role

Join a healthcare team dedicated to compassionate patient care as a Licensed Practical/Vocational Nurse (LPN/LVN). This role involves providing and documenting skilled nursing care under supervision, following care plans and physician orders.

Responsibilities

• Deliver quality nursing care per physician directives
• Monitor and report patient condition changes
• Document interventions, assessments, and responses accurately and timely
• Participate in care planning and updates
• Educate patients and families on disease processes and self-care
• Assess patient conditions and implement nursing actions
• Update care plans as needed
• Adhere to infection control and safety standards
• Maintain professional development through ongoing education

Requirements

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification
• TB test within the last 12 months
• Reliable transportation and ability to travel within the service area
• Physical ability to lift up to 50 pounds, reposition patients, and perform prolonged physical activities

Qualifications

• Preferred: six months of clinical nursing experience and home health background
