In-Home Healthcare LVN - Adult Patients (Day Shifts)

About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under supervision of a Registered Nurse.

Responsibilities

• Delivering and coordinating patient care per physician orders
• Assessing patient conditions and documenting activities
• Updating care plans
• Educating patients and families on disease management and self-care
• Recognizing changes in patient needs and implementing appropriate interventions
• Ensuring compliance with policies, regulations, and infection control standards

Requirements

• Active, unrestricted LPN/LVN license
• Current CPR certification
• TB test within the past year
• Clinical assessment and documentation skills
• Ability to work in diverse environments
• Physical ability to lift patients and equipment
• Adaptability to various home environments, including exposure to infectious materials
