In Home Healthcare LVN - Adult Patients (Day Shifts)

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse, ensuring quality patient care aligned with physician plans.

Responsibilities

• Assess patient conditions and document care activities
• Update care plans as needed
• Educate patients and families on disease management and self-care
• Adhere to infection control standards and compliance regulations
• Maintain professional accountability

Requirements

• Active, unrestricted LPN/LVN license
• Current CPR certification
• TB testing within the last 12 months
• Reliable transportation

Qualifications

Preferred candidates have at least six months of clinical experience and familiarity with home health care.
