In-Home Healthcare LVN - Adult Patients (Day Shifts)

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting. Responsibilities Include Delivering and documenting patient care according to physician orders and care plansAssessing patient conditions, recognizing changes, and reporting to supervisors or physiciansImplementing nursing actions to achieve expected outcomes and prevent risksEducating patients and families on disease management, self-care, and preventionAdhering to infection control and confidentiality standardsParticipating in ongoing education and maintaining compliance with policies and regulations Qualifications And Skills Active, unencumbered LPN/LVN license in the practice stateCurrent CPR certification and TB test within last 12 monthsPreferably six months of clinical experience; home health experience is a plusStrong attention to detail, organizational skills, and effective communicationAbility to travel, lift up to 50 pounds, and respond to emergency situations Additional Requirements Proficiency in clinical assessments and documentationAbility to work in diverse environments, including exposure to bodily fluids and infectious diseasesPhysical ability to perform patient repositioning and equipment movement
