About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings.
Responsibilities
- Deliver and document patient care according to care plans and physician orders.
- Monitor and assess patient conditions, reporting changes promptly.
- Update nursing care plans and implement appropriate nursing actions.
- Educate patients and families on disease management, self-care, and prevention.
- Ensure infection control and maintain compliance with policies and regulations.
Qualifications
- Active, unencumbered LPN/LVN license in the practice state (compact license transfer within 90 days if applicable).
- Current CPR certification and TB test within the last 12 months.
- Six months of hands-on nursing experience preferred.
- Reliable transportation required.
Skills
- Attention to detail, effective communication, and organization.
- Problem-solving skills and ability to manage time efficiently.
- Physical ability to lift up to 50 pounds, reposition patients, and perform mobility tasks.
- Ability to work in diverse environments, including exposure to blood and infectious materials.
