In-Home Healthcare LVN - Adult Patients (Day Shifts)

Responsibilities

• Provide and document skilled nursing care under supervision, following care plans and physician orders.
• Assess patient conditions continuously and report changes promptly.
• Update and implement nursing care plans to achieve desired outcomes.
• Educate patients and families on disease processes, self‑care, and prevention strategies.
• Adhere to infection control protocols and maintain compliance with regulations.

Qualifications

• Active, unrestricted LPN/LVN license in the practice state.
• Current CPR certification and TB test within the last 12 months.
• Proficiency in clinical assessments, documentation, and compliance.
• Preferred: Six months of recent clinical experience, home health background, and reliable transportation.

Physical and Environmental Requirements

• Ability to lift up to 50 pounds and reposition patients.
• Prolonged walking, standing, and bending in diverse environments.
• Ability to respond to emergency situations effectively.
