We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting. Key Responsibilities Include Delivering and documenting patient care according to care plans and physician orders.Assessing patient conditions, reporting changes, and implementing appropriate nursing actions.Participating in and updating nursing care plans to achieve desired outcomes.Providing education to patients and families on disease management, self-care, and prevention.Maintaining compliance with policies, regulations, and infection control standards.Utilizing electronic documentation tools for accurate record-keeping. Qualifications And Skills Active, unencumbered nursing license in the practice state.Current CPR certification and TB skin test within the last year.Six months of clinical nursing experience preferred; home health experience is a plus.Strong attention to detail, organization, communication, and problem-solving skills.Reliable transportation and ability to perform physical tasks such as lifting up to 50 pounds and repositioning patients. This role requires adaptability to various environments and compliance with health-related requirements, including vaccinations and testing as per jurisdictional mandates.
