We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home healthcare setting. Key Responsibilities Include Delivering and coordinating patient care according to physician orders and care plans.Monitoring and assessing patient conditions, reporting changes promptly.Documenting assessments, nursing actions, responses, and care activities accurately and timely.Implementing and updating nursing care plans, taking appropriate actions to achieve desired outcomes.Recognizing and responding to changes in patient needs to prevent risks.Educating patients and families on disease management, self-care, and prevention strategies.Maintaining infection control standards and complying with all regulatory requirements.Participating in ongoing education and skill development. Qualifications And Skills Active, unencumbered nursing license in the practice state.Current CPR certification.Reliable transportation and ability to travel to patient homes.Attention to detail, strong organization, communication, and problem-solving skills.Physical ability to lift up to 50 pounds and perform patient repositioning.Proficiency in clinical assessments, documentation, and compliance with policies. Additional Requirements Include Six months of hands-on nursing experience preferred.State-specific experience requirements for certain regions.Ability to operate in diverse environments, with exposure to bodily fluids and infectious diseases.
