About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting.
Responsibilities
- Deliver and document patient care according to physician orders and care plans.
- Assess patient condition, monitor changes, and report to supervisors or physicians.
- Educate patients and families on disease management, self-care, and prevention.
- Maintain confidentiality and adhere to professional and regulatory standards.
- Implement infection control measures and ensure safety during care delivery.
Qualifications and Skills
- Active, unrestricted LPN/LVN license in the practice state.
- Current CPR certification and TB test within last 12 months.
- Proficiency in clinical assessments, documentation, and compliance.
- Reliable transportation and physical ability to lift up to 50 pounds, reposition patients, and perform necessary mobility tasks.
- Attention to detail, strong communication, time management, and problem-solving skills.

- Experience in clinical or home health settings preferred.
- Ability to work independently in diverse environments and respond effectively to emergencies.
