About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse.
Responsibilities
- Delivering and coordinating patient care according to physician orders
- Assessing and monitoring patient conditions
- Documenting activities and responses
- Updating care plans
- Educating patients and families on disease management and self-care techniques
- Maintaining confidentiality, adhering to safety and infection control standards, and ensuring compliance with regulations and policies
Qualifications
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification
- TB skin test within the past 12 months
- Six months of hands-on nursing experience preferred
- Reliable transportation
