About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings.
Responsibilities
- Delivering and documenting patient care per physician orders and care plans.
- Assessing patient condition, reporting changes, and implementing appropriate interventions.
- Educating patients and families on disease management, self-care, and prevention.
- Ensuring compliance with policies, regulations, and infection control standards.
- Participating in ongoing education and maintaining clinical competencies.
Qualifications and skills
- Active, unrestricted LPN/LVN license in the practice state.
- Current CPR certification and TB test within last 12 months.
- Preferred: six months of recent clinical experience, home health background.
- Strong attention to detail, organization, communication, and problem-solving skills.
- Ability to lift 50 pounds, travel, and respond to emergency situations.
