Part-Time Clinician (Nurse Practitioner or Physician Assistant) responsible for providing in-home and virtual health evaluations to patients in their own homes, focusing on building trusted relationships and improving health outcomes.

Responsibilities

• Conduct comprehensive health assessments, including medication and medical history reviews, physical evaluations, and diagnostic tests as ordered by health plans.
• Use digital tools like iPads to streamline evaluations without prescribing medications or altering treatments.
• Engage with patients to address health questions and promote personalized care.
• Travel locally and within licensed states as needed.
• Provide diagnostic and preventive services, including screenings and tests.

Qualifications

• Active, unrestricted licenses in coverage areas.
• Board certification (required).
• Preferred multi-state licenses.
• Ability to cover primary locations such as Hartford, CT.

Compensation

Competitive pay range: $46.03 – $99.14/hr
