About the role
Provide in-home health evaluations to improve patient outcomes as a Part-Time Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conduct 45-minute in-home health assessments, including medical history review, physical evaluation, and optional diagnostic tests.
- Utilize an iPad and clinical workflow; no prescribing, ordering labs, or changing treatments required.
- Build trusted relationships with patients and connect them to appropriate care.
- Perform virtual visits as needed and travel within licensed areas.
- Deliver Diagnostic & Preventive Services like spirometry, bone density screening, etc.
Requirements
- Active, unrestricted licenses in designated areas.
- Board certification required; multi-state licenses preferred.
- Ability to work in diverse home environments with pets, smoke, or allergens.
