About the role
We are seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations focused on improving patient health outcomes.
Responsibilities
- Perform comprehensive assessments including medication reviews, physical evaluations, and diagnostic tests as ordered, using an iPad for clinical workflows.
- Build trusted relationships with patients in their homes, providing personalized care and connecting them to appropriate services.
- Visit patients at home or virtually and conduct diagnostic procedures.
- Adhere to safety standards in diverse environments.
Requirements
- Active licensure and board certification.
