About the Role
A Part-Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient outcomes.
Responsibilities
- Conduct 45-minute visits including medical history review, physical assessment, and optional diagnostic tests.
- Engage with patients in their homes, build trusted relationships, and deliver personalized health evaluations.
- Perform virtual visits as needed and provide diagnostic services such as spirometry and screenings.
- Travel locally within licensed states and work flexibly to meet patient needs.
- Connect patients with appropriate care and utilize clinical workflows on iPad.
- Support health improvement initiatives and offer comprehensive preventive services.
- Contribute to community health.
Requirements
- Active, unrestricted licenses and board certification.
- Multi-state licenses preferred.
- Ability to work in diverse home environments and perform assessments safely.
- Strong communication skills and compassion are essential.
