In-Home Health - Nurse Practitioner or Physician Assistant (Part Time)

About the Role

A Part-Time Clinician (Nurse Practitioner or Physician Assistant) provides in-home health evaluations to patients, fostering trusted relationships and improving health outcomes.

Responsibilities

• Conduct 45-minute evaluations including medication and medical history reviews, physical exams, and diagnostic tests if needed.
• Utilize an iPad and clinical workflow; no prescribing, lab ordering, or treatment modifications required.
• Engage with patients virtually or in-person, offering personalized health assessments and connecting them to appropriate care.
• Travel locally within licensed states as needed.
• Offer diagnostic and preventive services, such as spirometry and screenings.

Requirements

• Active, unrestricted licensure in the coverage area(s).
• Board certification (required).
• Preferred multi-state licenses.
• Ability to cover primary locations, e.g., Hartford, CT.

Pay

$46.03 - $99.14 hourly, based on experience and location.
