In-Home Health - Nurse Practitioner or Physician Assistant (Part Time)

About the Role

We are seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations, providing personalized assessments in patients' homes. This role involves reviewing medical and medication histories, performing physical evaluations, and ordering diagnostic tests as needed using an iPad-based workflow. Clinicians build trusted relationships, engage with patients, and deliver comprehensive care without prescribing medication or altering treatments. Flexibility to travel locally and conduct virtual visits is required.

Responsibilities

• Perform health evaluations and diagnostic services at patients' homes or virtually
• Build rapport and provide education to improve health outcomes
• Complete visits efficiently while ensuring quality and patient safety
• Travel within licensed states as needed
• Work comfortably in diverse home environments, including potential exposure to household pets and allergens

Requirements

• Active, unrestricted license as a Nurse Practitioner or Physician Assistant
• Board certification
• Multi-state licenses preferred
• Strong communication skills and empathy
• Ability to work independently
