In-Home Health - NP or PA (Full Time) - St. Lawrence/ Plattsburgh, NY

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Responsibilities

• Conduct in-home health assessments for patients
• Build trusted relationships with patients during visits
• Answer patient health questions and provide education
• Utilize an iPad and clinical workflow for evaluations
• Perform diagnostic tests as ordered by health plans

Requirements

• Active, unrestricted license in the coverage area
• Board certification required
• Multi-state licenses preferred
• Willingness to travel locally and regionally
• Experience in providing in-home health evaluations

Pay

$95,738 – $206,206 annually
