In-Home Healthcare Provider (NP/PA) - Full-Time

Responsibilities

• Conduct comprehensive health evaluations for Medicare and vulnerable populations in patients' homes
• Perform detailed health risk assessments and review medications
• Identify and address care gaps, educate patients on chronic disease management and prevention
• Utilize technology and data tools to document encounters and support value-based care
• Collaborate with remote clinical teams to improve patient outcomes

Qualifications & Skills

• Licensed Nurse Practitioner or Physician Assistant
• Experience in in-home or community-based healthcare preferred
• Strong communication and patient education skills
• Ability to work independently with flexibility
• Comfort with technology and data documentation
