Hospital at Home - Full-Time NP/PA

About the role

Advocate Health is seeking an Acute Care Nurse Practitioner or Physician Assistant to join the Hospital at Home team at Navicent Medical Center. This full-time hybrid opportunity supports the expansion of the Hospital at Home program. The APP will deliver acute inpatient-level care through a combination of virtual care and in-home patient visits while collaborating with physicians and a multidisciplinary healthcare team to provide exceptional, patient-centered care.

Responsibilities

• Performs initial and periodic medical histories, physical examinations, and daily patient assessments.
• Records findings of examinations, assessments, and plan of care.
• Performs minor medical and/or surgical procedures within scope of practice.
• Writes appropriate prescriptions and administers medications.
• Orders and interprets laboratory and radiology examinations.
• Determines and implements appropriate treatment plans.
• Documents patient encounters, interventions, and observations accurately in the electronic medical record.
• Acts as a patient advocate for the population served.
• Participates in interdisciplinary meetings, quality improvement initiatives, and development of policies and procedures as requested.

Requirements

Nurse Practitioner

• Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program required.
• Current license to practice as a Registered Nurse and Advanced Practice Registered Nurse in the applicable state required.
• National board certification appropriate to specialty required (ACNP, AGACNP, AGNP, PNP, or PNP-AC based on population served).
• Must have or be eligible for a DEA license.
• Eligible for prescriptive authority application through the applicable Board of Nursing as required.
• Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.

Physician Assistant

• Graduate of an accredited Physician Assistant program required.
• Current Physician Assistant license in the applicable state required.
• NCCPA certification required.
• Must have or be eligible for a DEA license.
• Eligible for prescriptive authority application through the applicable Medical Board as required.
• Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
