About the role

The role combines virtual patient care with in-home visits throughout the service area.

Responsibilities

  • Performs initial and periodic medical histories, physical examinations, and daily patient assessments.
  • Records findings of examinations, assessments, and plan of care.
  • Performs minor medical and/or surgical procedures within scope of practice.
  • Writes appropriate prescriptions and administers medications.
  • Orders and interprets laboratory and radiology examinations.
  • Determines and implements appropriate treatment plans.
  • Documents patient encounters, interventions, and observations accurately in the electronic medical record.
  • Acts as a patient advocate for the population served.
  • Participates in interdisciplinary meetings, quality improvement initiatives, and development of policies and procedures as requested.

Requirements

  • Nurse Practitioner: Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program required. Current license to practice as a Registered Nurse and Advanced Practice Registered Nurse in the applicable state required. National board certification appropriate to specialty required (ACNP, AGACNP, AGNP, PNP, or PNP-AC based on population served). Must have or be eligible for a DEA license. Eligible for prescriptive authority application through the applicable Board of Nursing or Medical Board as required. Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
  • Physician Assistant: Graduate of an accredited Physician Assistant program required. Current Physician Assistant license in the applicable state required. NCCPA certification required. Must have or be eligible for a DEA license. Eligible for prescriptive authority application through the applicable Medical Board as required. Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.

Schedule

This is a full-time hybrid position supporting the Hospital at Home program at Navicent Medical Center. The schedule consists of a mixed rotation of day shifts, including weekends, swing shifts, holidays, and weekend coverage based on operational needs.
