In-Home Healthcare Provider (NP/PA) - Full-Time

Responsibilities

• Perform comprehensive in-home health evaluations for diverse and vulnerable patient populations.
• Review medical histories and conduct detailed physical examinations.
• Identify care gaps and document findings using a technology-enabled platform to support value-based care.
• Collaborate with interdisciplinary teams to provide patient education and coordinate follow-up care.
• Utilize data-driven insights to enhance patient outcomes.

Qualifications & Skills

• Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience.
• Strong assessment, documentation, and communication skills.
• Ability to work independently and as part of a team in a fast-paced, technology-driven environment.
