In-Home Nurse Practitioner or Physician Assistant (Full Time) - PA
About the role
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, helping bridge gaps in care by building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations.
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present varying conditions, including household pets, secondhand smoke, or other airborne allergens.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
- Perform additional diagnostic tests as ordered by health plans (e.g., diabetic eye exams, spirometry, bone density screening).
- Travel locally and within licensed states as needed.
- Conduct virtual visits when required.
- Provide education and guidance to health plan members to improve their understanding of health and care options.
- Build trusted relationships with members to support better health outcomes.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the specified primary locations.

Pay
The typical pay range for this role is $87,035.00 - $187,460.00 annually, depending on experience, education, geography, and other relevant factors.
Schedule
- Full-time position with anticipated weekly hours of 40.
