In-Home Nurse Practitioner or Physician Assistant (Full Time)

Responsibilities

• Perform comprehensive health assessments for vulnerable patients in their homes
• Conduct clinical evaluations and review medications
• Identify gaps in care and document findings using the organization’s technology platform
• Collaborate with an interdisciplinary team to develop care plans
• Coordinate follow-up services and utilize data-driven insights to enhance patient outcomes

Qualifications & Skills

• Current Nurse Practitioner or Physician Assistant licensure
• Strong clinical assessment and evaluation skills
• Ability to work independently with autonomy
• Excellent communication and teamwork abilities
• Proficiency in documenting clinical findings and using healthcare technology
