In-Home Health - NP/PA (Full Time) - Bronx, New York

You will partner with cross-functional stakeholders to clarify priorities, solve operational challenges, and move important work forward. Location: Remote - US based candidates only, no visa sponsorship available.

Responsibilities

• Engage in one-on-one health evaluations in patients' homes
• Build trusted relationships to improve patients' health outcomes
• Conduct thorough medication and medical history reviews
• Complete physical evaluations and administer diagnostic tests as ordered
• Travel locally and within licensed states for patient visits
• Facilitate virtual visits when necessary
• Deliver comprehensive diagnostic and preventive services

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
