In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Bronx, NY

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Responsibilities

• Conduct in-home health evaluations to assess patients' medical histories
• Engage patients in discussions to address health questions and needs
• Perform physical evaluations as needed during visits
• Provide connections to appropriate care based on individual needs
• Conduct virtual visits to accommodate patients
• Utilize an iPad and clinical workflow for evaluations

Requirements

• Active, unrestricted licenses in coverage area(s)
• Board certification required
• Multi-state licenses preferred
• Ability to cover the Bronx and surrounding area

Pay

$95,738 – $206,206 annually
