In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Oneida Syracuse, NY

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.

Responsibilities

• Conduct in-home health evaluations to assess patient health and needs
• Engage patients in personalized health discussions
• Use iPad and clinical workflow tools during evaluations
• Bridge gaps in care by connecting patients to necessary services
• Travel locally and within licensed states as needed
• Perform virtual visits when necessary
• Provide diagnostic and preventive services as indicated

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Ability to cover Oneida, Syracuse, NY and surrounding area

Pay

$95,738 – $206,206 annually
