Nurse Practitioner or Physician Assistant, Northern Syracuse -Care at Home

About the Role

This position involves providing in-home healthcare services as part of an interdisciplinary team, focusing on preventive care to help patients stay well at home. The role requires daily travel to patient residences within an assigned area, with no night, weekend, or holiday shifts.

Responsibilities

• Conduct comprehensive in-home assessments
• Manage acute and chronic health conditions
• Coordinate care with interdisciplinary team members
• Document clinical information accurately
• Engage in advanced care planning and patient education
• Deliver holistic, patient-centered care in diverse environments

Requirements

• Active licensure and certification (e.g., ANCC, AANP, NCCPA)
• Current DEA license or ability to obtain
• Reliable transportation
• Proficiency with electronic medical records
• At least one year of clinical experience in settings such as geriatrics, home health, or long-term care (preferred)
• Experience with complex or geriatric populations (preferred)

Pay

Compensation ranges from $125,000 to $175,000, including base pay and bonuses.
