Nurse Practitioner or Physician Assistant - In Home-Care-Manhattan

About the Role

This position involves providing in-home care to high-risk patients, focusing on comprehensive assessments, managing acute and chronic conditions, and conducting advanced care planning. The role requires daily travel within the assigned area to patients' homes, ensuring accurate documentation and collaborating with an interdisciplinary team. It offers a supportive environment centered on preventive care to help patients stay well at home, with no night, weekend, or holiday shifts.

Responsibilities

• Conduct thorough home assessments, including physical exams, medication reconciliation, and screenings
• Manage acute episodes, chronic conditions, and sub-acute rehabilitation
• Document clinical findings accurately and collaborate with team members
• Engage in advanced care planning and patient education

Requirements

• Active state licensure and DEA registration (if applicable)
• Certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (for Nurse Practitioners)
• Current NCCPA certification and licensure for Physician Assistants
• Reliable transportation and a valid driver’s license
