About the Role
Provide comprehensive in-home care to high-risk patients as part of an interdisciplinary team, focusing on preventive and acute care management. Travel within assigned areas is required, with no night, weekend, or holiday shifts.

Requirements
- Active licensure and certification (NP or PA) with prescriptive authority
- Ability to obtain collaborative agreements if needed
- Reliable transportation and active driver’s license

- Preferred experience in geriatrics, chronic illness, or home health
- Proficiency in electronic medical records
