About the role

Middletown, New York is seeking a Nurse Practitioner to join their team in Orange County, NY. Optum is a clinician-led care organization that is changing the way clinicians work and live.

Primary Responsibilities

  • Provide care in the patient’s home setting.
  • Manage 5-8 patient visits per day, including home visits and potentially video visits for established patients.
  • Prioritize patient goals through regular interdisciplinary meetings.
  • Collaborate with the patient’s Primary Care Physician to develop a care plan.
  • Diagnose and treat acute, chronic, and long-term healthcare issues, in collaboration with the patient’s Primary Care Physician.
  • Evaluate and address disease prevention and health promotion issues while administering quality patient care.
  • Maintain accurate medical records and confidential medical records, documenting all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations.
  • Educate patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.

Required Qualifications

  • Advanced Nurse Practitioner Certification in Primary Care, Geriatric, Adult, with unrestricted licensure in the state of NY at time of employment.
  • Current and unrestricted DEA certificate required or ability to obtain.
  • Valid driver's license.
  • Ownership of a reliable and insured vehicle, with the ability to travel within assigned territory to meet with patients.
  • Comfortable working independently in a home-visit setting and clinic setting.
  • Ability to communicate with patients, primary care team members, and management staff.
  • Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician.
  • Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice.
  • Advanced assessment and treatment plan implementation skills.

Preferred Qualifications

  • Effective communication skills.
  • Effective organization skills and ability to multi-task.
  • Takes Initiative, creative, has problem solving ability, is adaptable, and flexible.
  • Ability to work without direct supervision and practice autonomously.
  • Geriatric care experience.
  • Knowledge of mental health issues.
  • 1+ years of Outpatient Clinic experience.
  • Familiarity with Medicare and ICD-10 coding.
