About the Role
For our client, we are seeking a In-Home Health - Nurse Practitioner Or Physician Assistant (full Time) - Steuben/ Elmira NY to join the team of a leader in the Healthcare space. This role will support clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.
Location
Remote - US based candidates only, no visa sponsorship available
Compensation
$95,738 – $206,206 annually
Responsibilities
- Conduct in-home health evaluations for patients
- Engage with patients to answer health questions and provide education
- Review medication and medical histories during visits
- Perform physical evaluations and diagnostic tests as needed
- Document clinical findings using an iPad and clinical workflow
Qualifications
- Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage area
- Board certification is required
- Multi-state licenses are preferred
- Ability to travel locally and within licensed states
- Experience with in-home patient evaluations or screenings
