In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Snohomish, WA

Responsibilities

• Conduct in-home health evaluations and engage with patients
• Build trusted relationships to improve health outcomes
• Perform medication and medical history reviews
• Handle virtual visits as necessary
• Provide diagnostic and preventive services products

Requirements

• Active, unrestricted license in coverage areas (Snohomish)
• Board certification required
• Multi-state licenses preferred
• Experience in patient engagement and health evaluations
• Strong communication and interpersonal skills

Pay

$95,738 – $206,206 annually
