In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Kennewick, WA

You will partner with cross-functional stakeholders to clarify priorities, solve operational challenges, and move important work forward. Location: Remote – US-based candidates only, no visa sponsorship available.

Responsibilities

• Engage with patients during in-home health evaluations
• Conduct medication and medical history reviews
• Perform physical evaluations and diagnostic tests as needed
• Travel locally to provide services in patients' homes
• Facilitate virtual visits when required
• Champion connections to appropriate care based on individual needs
• Collaborate with a network of clinicians focused on health outcomes

Requirements

• Active, unrestricted NP or PA license in coverage areas
• Board certification required
• Preferred multi-state licenses
• Ability to cover primary locations in Kennewick/Benton County
