In-Home Health Clinician - Nurse Practitioner or Physician Assistant (Part Time)

Role Overview: The Part Time Clinician (Nurse Practitioner or Physician Assistant) provides in-home health evaluations to help bridge gaps in care and improve health outcomes. These evaluations, lasting about 45 minutes, include reviewing medication and medical history, conducting physical assessments, and performing diagnostic tests as ordered. The role involves engaging with patients in diverse home environments, using digital workflows, and conducting virtual visits when needed.
