About the Role

The Oregon Department of Human Services seeks a Human Services Case Manager to match the needs of Aging and People with Disabilities in the community with available federal, state, and local programs or resources. This role involves case management, screening applicants for long-term community-based care and nursing homes, assessing eligibility, redetermining needs, adjusting benefits and services, and advocating for clients within the community. Bilingual Spanish/English is preferred but not required; language testing is required to receive a 5% language differential.

Responsibilities

  • Match client needs with available federal, state, and local programs or resources.
  • Provide case management for Aging and People with Disabilities.
  • Screen applicants for long-term community-based care and nursing homes.
  • Assess eligibility, redetermine needs, and adjust benefits and services accordingly.
  • Act as an advocate for clients and the Aging and People with Disabilities program within the community.
  • Interpret and implement policy and procedures.
  • Prepare and maintain written narrative documentation of actions taken.
  • Conduct interviews to obtain information and evaluate next steps.
  • Visit clients in homes, hospitals, nursing homes, APD offices, and other provider locations.

Requirements

  • A Bachelor’s degree in Behavioral Science, Social Science, or a closely related field; OR
  • A Bachelor’s degree in any field and one year of human services-related experience; OR
  • An Associate’s degree in Behavioral Science, Social Science, or a closely related field and two years of human services-related experience; OR
  • Three years of human services-related experience.

Human services-related experience includes work providing assistance to individuals and groups with issues such as economically disadvantaged populations, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or inadequate housing.

  • Experience managing services for individuals.
  • Experience interpreting and implementing policy and procedures.
  • Experience preparing and maintaining written narrative documentation.
  • Experience interviewing to obtain information and evaluate next steps.
  • Current driver’s license and acceptable driving record.
