About the role
The Oregon Department of Human Services is seeking a Case Manager to support older adults and people with disabilities in obtaining the long-term care services they need. This role involves assessing needs, developing service plans, and coordinating services to ensure continuity of care.
Responsibilities
- Assessing Oregonians applying for or currently receiving long-term care services
- Developing, coordinating, and arranging a variety of services to support older adults and people with disabilities
- Providing choice counseling and helping establish and implement case management plans
- Collaborating with the eligibility team and utilizing the ONE System to review and update Long Term Care Services
- Interviewing Oregonians to determine eligibility for healthcare, social services, and protective services
- Conducting interviews in person or on the phone using the Case Assessment and Planning System (CA/PS) assessment tool
- Processing applications, inquiries, and data according to policy timeframes
- Maintaining and updating social service plans and identifying and mitigating safety risks
- Contacting Oregonians or collateral contacts monthly to monitor service plans and safety needs
- Ensuring service delivery is culturally competent and trauma-informed
- Collaborating with Adult Protective Services, AFH Licensors, and other agencies to ensure the safety and protection of Oregonians
- Ensuring mandatory reporting guidelines are followed
Requirements
- Three years of human services related experience OR a bachelor's degree in a Behavioral Science, Social Science, or a closely related field OR an associate degree in a Behavioral Science, Social Science, or a closely related field AND two years of human services related experience
- Experience coordinating services and support for individuals by assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes as a case manager
- Experience interpreting and applying laws, regulations, policies, or procedures to make consistent, accurate, and well-supported decisions
- Experience preparing clear, accurate, and timely documentation that supports decisions, communicates actions taken, and complies with organizational or legal requirements
- Experience conducting interviews or information-gathering conversations to assess needs, evaluate available information, and determine appropriate next steps using sound judgment as a case manager
