About the Role

The Oregon Department of Human Services is hiring a Case Manager (Human Services Case Manager) to join the Aging & People with Disabilities (APD) team in Hillsboro, Oregon. In this position, you will assess people applying for or currently receiving long-term care services to develop, coordinate, and arrange services that allow older adults and people with disabilities to get the continuity of care they need.

Summary of Duties

  • Conduct interviews with participant(s) and collateral contacts to determine initial and ongoing eligibility for long-term care, healthcare, social services, and protective services.
  • Utilize state systems, assessment tools, and available resources to document, review, and update case information and long-term care services.
  • Assess individual needs, safety, and risk to develop detailed, person-centered service plans and determine appropriate services, referrals, and resources.
  • Process applications, inquiries, and eligibility information within established policy timeframes, ensuring accurate documentation, forms, and communication of eligibility decisions.
  • Contact participants or collateral contacts monthly to monitor service plan and safety needs.
  • Work closely and collaboratively with the participant, other agencies, medical providers, and community partners to provide benefits, services, and information.
  • Work with participant to create person-centered risk mitigation plans and provide risk mitigation as needed.

Minimum Qualifications

  • A valid driver's license and acceptable driving record.
  • Three years of human services related experience (work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field; OR
  • An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience; OR
  • An equivalent combination of experience, education, and training.

Essential Attributes

  • Experience managing a caseload and coordinating services and resources to meet individual client needs.
  • Experience managing competing deadlines and priorities while adhering to established policies and procedures.
  • Experience writing documents or reports that include a conclusion and recommended action (examples: case plans, court reports, investigative reports).
  • Experience conducting case management interviews to gather and assess information, evaluate needs, and determine appropriate next steps.
  • Experience shifting organizational needs, priorities, timelines, and circumstances while maintaining productivity, professionalism, and sound decision-making.
  • Ability to recognize trauma, reduce traumatization, and ensure interactions and services are safe, equitable, and respectful.
  • Preference given to candidates who are bilingual in Spanish and English.

Working Conditions

  • General office environment in a cubicle setting.
  • Work is conducted both in office setting and in long-term care facilities and the community at large.
  • May be required to work a flexible work schedule as determined by the hiring supervisor.
  • Fast-paced environment with constantly evolving policies and procedures.
  • Travel is required to meet business needs and to attend employee training classes.
  • May drive a variety of vehicles to remote places in varied terrain and possibly inclement weather.
  • May come in contact with animals, some of which may be dangerous.
  • May be exposed to communicable diseases, such as flu virus, Covid-19, and MRSA.
  • Will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions.
  • May encounter stressful situations that require quick decision-making to ensure safety of yourself and others.
