Case Manager (Human Services Case Manager) Bilingual Preferred (Spanish & English)
About the role
The Oregon Department of Human Services seeks a Case Manager to support older adults and people with disabilities. This role involves assessing and arranging services, developing case management plans, and ensuring compliance with regulations.
Responsibilities
- Assessing and arranging long-term care services for eligible individuals
- Developing, coordinating, and implementing case management plans
- Maintaining and updating eligibility for long-term care services
- Monitoring social service plans and identifying safety risks
- Contacting Oregonians monthly to review and update service plans
- Conducting interviews and providing case planning services
- Collaborating with Adult Protective Services and other agencies
- Ensuring compliance with mandatory reporting guidelines
Requirements
- Three years of human services related experience or a combination of education and experience
- A bachelor's degree in a Behavioral Science, Social Science, or a closely related field, or an associate degree with two years of relevant experience
- Experience coordinating services and supporting individuals with various issues
- Ability to interpret and apply laws, regulations, and policies accurately
- Proficiency in preparing clear and accurate documentation
- Experience conducting interviews and gathering information to assess needs
Qualifications
- Bilingual fluency in Spanish and English preferred
- Experience working with diverse communities and populations
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Skills
- Excellent written and verbal communication skills
- Knowledge of long-term care services and eligibility criteria
- Ability to work in a fast-paced, collaborative environment
- Proficiency in Microsoft Office and the ONE System

Pay
$5,193 - $6,904 annually
Schedule
8:00 a.m. – 5:00 p.m., Monday – Friday Pacific Standard Time (PST)
