About the role
Serve as a Home Health Physical Therapist Assistant, providing personalized rehabilitation care in patients' homes. Collaborate with the care team to implement physical therapy treatment plans, including manual exercises, therapeutic modalities, and other interventions. Document patient progress, report observations, and participate in care coordination and discharge planning to support patient recovery and independence.
Responsibilities
- Administer physical therapy treatments per the plan developed by the Physical Therapist
- Maintain accurate documentation of patient progress and clinical notes
- Contribute to care coordination, discharge planning, and act as a resource for healthcare team members
Qualifications & Skills
- Current and unrestricted PTA license in the relevant state
- Valid driver’s license, auto insurance, and reliable transportation
- CPR certification
- Preferred: Home health experience

Pay
Compensation varies based on experience, with a pay range of $41.00 – $57.00 per visit/unit or $64,000 – $87,500 annually for full-time.
Schedule
The role requires participation in driver safety and TB screening programs.
